Job Description

The Hennepin County Public Health Clinic (PHC) is seeking a Registered Nurse (RN) who has an interest in and experience working with a diverse population in a public health setting. This position will provide clinical support services to clients and families of refugees, and those dealing with tuberculosis.
The RN in this position will provide Refugee health assessment and screening for newly arrived refugees to Hennepin County, identify their ongoing health & support needs and connect them to resources. This position will be part of a multi-disciplinary team that provides primary and acute care services to a wide variety of patients.

Location and hours:
This position is located at the Health Services Building at 525 Portland Avenue South, Minneapolis MN 55415. Hours are primarily 8 a.m. to 4:30 p.m. Once the clinic resumes evening hours, this position will require working one evening shift a week on Mondays or Wednesdays until 7:30 p.m. No overnight or holiday shifts are required. The person in this position will be cross trained to assist in other services areas within the clinic.

In this position, you will:

  • Deliver nursing care and interventions, working under authority of independent nurse scope of practice, delegated care or program standing orders. 
  • Provide triage, assessment, screening, and nurse visits to clients needing refugee screening and assessment.
  • Provide health education to newly arrived refugees to Hennepin County and provide community resources.
  • Coordinate efforts of an interdisciplinary patient care team, directing workflow, while facilitating communication between care team members to ensure integrated and coordinated care.
  • Provide primary care coordination and referral to ensure seamless delivery of care and care transitions, to improve communication among PHC care teams, and guide patients and communities toward an effective use of resources.
  • Document data, plans, clinical actions, client progress, response to care and other relevant client/patient information in compliance with legal statute and rules of confidentiality. 
  • Communicate and coordinate with members of the care delivery team, clients, families, and other outside resources to ensure delivery of medical care and successful care transitions. 
Need to have:
  • Graduation from an accredited school of nursing and six months or more of nursing experience in a related area.
  • A valid license, in good standing, as a Registered Nurse, or a permit to practice, issued by the State of Minnesota.
  • Current CPR certification.
Nice to have:

  • Public Health Nurse (PHN) certification.
  • Experience:
    • As a Registered Nurse in a clinic providing services to the medically underserved and marginalized populations.
    • Using the Epic and/or other electronic health record system.
    • Working with newly arrived refugee families, and patients experiencing housing instability.
    • Working in a primary care clinic, specialty clinic, or performing care coordination duties.
  • Knowledge of:
    • Theory, techniques, and practices of professional nursing; preparation and administration of medications, treatments and medical procedures, counseling and instructional techniques.
    • Sensitivity to the needs and issues of diverse populations that may present with social vulnerabilities and have a history of experiencing barriers to health care such as mental health, opioid addiction, poverty or unstable housing.
    • Hennepin County community and Refugee service resources.
  • Ability to
    • Work with multiple providers, staff, and collaborative partners with differing needs and opinions to improve the care to diverse and underserved people with a variety of cultural and language barriers.
    • Provide professional nursing services including triage, assessment, and providing direct nursing care interventions.
    • Deal objectively with individuals whose attitudes and behaviors differ from him/herself.
    • Provide direct nursing care to children, youth, adults, and seniors.
