About the Role:
We are looking for a passionate and detail-oriented Clinical Pharmacist to join our team! If you hold a PharmD degree and have experience working with HEDIS medication adherence initiatives, we'd love to have you on board. In this role, you'll help improve medication adherence, support patient outcomes, and ensure compliance with Medicare and HEDIS quality measures.
Key Responsibilities:
HEDIS Quality Measures:
Lead initiatives to enhance medication adherence and other pharmacy quality measures, including but not limited to antihypertensive, cholesterol-lowering, and diabetes medications.
Analyze medication adherence data, identify care gaps, and implement targeted strategies to improve adherence rates.
Collaborate with healthcare providers, care managers, and other team members to develop and execute plans that encourage adherence to prescribed therapies.
Patient Engagement:
Conduct Medication Therapy Management (MTM) sessions to address adherence barriers, provide patient education, and ensure understanding of medication regimens.
Work directly with patients to resolve issues related to medication adherence, including cost concerns, side effects, and complex dosing schedules.
Use motivational interviewing and other patient engagement techniques to help patients stay on track with their medication plans.
Data Analysis & Reporting:
Track HEDIS performance metrics and generate reports to evaluate the effectiveness of adherence initiatives.
Utilize electronic health records (EHR) and pharmacy data to identify at-risk patients and develop action plans for improving adherence.
Regularly update leadership on progress toward HEDIS goals and provide recommendations for improvements.
Collaboration & Training:
Serve as a subject matter expert in medication adherence within the organization.
Provide training and support to other pharmacists, care managers, and clinical staff on best practices related to medication adherence.
Participate in cross-functional teams focused on quality improvement, population health management, and value-based care initiatives.
Qualifications:
What You'll Need:
Doctor of Pharmacy (PharmD) degree from an accredited school of pharmacy.
Active pharmacist license in Georgia.
Minimum of 2 years of experience working with HEDIS medication adherence, quality improvement, or population health.
Strong knowledge of HEDIS measures, particularly those related to medication adherence.
Proficiency in EHR systems and data analysis tools.
Excellent communication, problem-solving, and organizational skills.
Ability to work independently and collaboratively in a team-oriented environment.
Experience with motivational interviewing or other patient engagement techniques is a plus.
What We'd Love You to Have:
Experience in a managed care setting, pharmacy benefits management (PBM), or healthcare quality improvement role.
Certification in Medication Therapy Management (MTM), Certified Geriatric Pharmacist (CGP), or other relevant credentials.
Understanding of value-based care models and their impact on medication adherence.
Additional Information:
Compensation:
Salary range for this role is $123K, with pay distributed bi-weekly.
Final compensation will be based on factors such as experience, qualifications, and location.
